Arranging access to health records

 

Data protection legislation entitles an individual to obtain a copy of information contained in their health care record.

This is commonly referred to as a subject access request and gives you the right to obtain a copy of your personal data including the information contained in your Health Care Record


 

If you would like to obtain a copy you may submit your request in writing, by email or verbally via our office team. Contact details are shown below:

To help us process your request faster, please include your name and address for correspondence and describe the information that you are requesting, giving us enough details about the information to allow us to correctly identify and locate it. You will also need to provide proof of your identity.

We must act on a subject access request without undue delay and at the latest within one month of receipt.

 

 

You should be aware that in certain circumstances your right to see some details in your health care record may be limited in your own best interests.

 

 

Your friends, family or representatives are not allowed to ask to see your records without your written consent.

 

 

You have the right to request that any inaccuracies be corrected or annotated.

By law, everyone working for or on behalf of IMS or other qualified health care providers must respect your confidentiality and keep your information secure.

We have a duty to protect your personal information and confidentiality and we take our responsibilities very seriously.

We are committed to taking all reasonable measures to ensure the confidentiality and security of personal data, whether it is in digital form or on paper.

Everyone working for IMS must comply with Data Protection legislation and the Common Law Duty of Confidentiality.

Information you provide to us will be used in confidence and only for the purposes explained to you and to which you consented, unless there are other circumstances covered by the law.
